Slough District Nursing service is inviting Qualified Nursing Associates with a registered NMC qualification to join our District Nursing Service. You will be utilising your fantastic skills, delivering top quality care and building rapport with our patients in their home. This is not your average nursing role, as our community nursing service offers a fantastic work life balance, with our shift pattern meaning you only work1 weekend per month! Our working hours/days are as follows: Monday to Friday (Core hours) - 08:30 – 16:30 1 weekend every 4 weeks with days off in the week Late shift – 11:00-19:00 Early shift – 07:00 – 15:00 We offer some fantastic perks, such as flexible working, part time offerings, incredible training and support programmes, preceptorship programmes and the highest paid mileage in the area. No day will be the same, as you will be doing anything from wound care to end of life care. Below are just some of the duties that you will be doing. • To provide care according to the available evidence of best practice. • Deliver clinical and therapeutic care to patients within agreed trust policies/ protocols, as documented in the band 4 competency assessment tool. • Develop, implement and evaluate nursing plans in response to identified needs of the individual, carers and community in partnership with clients • To work on own initiative and within scope of competence, referring appropriately to the Community Nursing Sister, Community Matron or other professionals as appropriate. • Support and teach patients to self-manage their condition where possible and empower carers to actively participate in the treatment plan. • Maintain excellent communication with patients, relatives and members of the multidisciplinary team regarding all aspects of care demonstrating a variety of communication skills in accordance with client group. • To be involved in the management of End of Life care and work in partnership with health, social and voluntary agencies to ensure patients are treated with dignity and respect, and to support their families and carers. • Prepare the patient and their family/carer for changes in the patient’s condition and support choice for End of Life care.
